    Age Estimation Of Dental Patients Using Orthopantomographs

    Abstract

    Identification of a person and age estimation has been an integral part of forensic and forensic dentistry.
    Several methods in techniques have been described to estimate patients age from dental records including study of OPGs (Orthopantomographs). The objective of this study was to determine age estimation from OPG and to find out if they properly correlated with the actual age of the patients. Two dentists examined 150, each 75, and indicated patients age. Third dentist compared their scores with the chronological age of the patients. The first dentist predicted 81.33% correctly and the second one 84%. Overall prediction rate was 82.66%. No significant difference was found in between them (p>0.05). Though it was found that the most accurate age estimation occurred in the age group of I (1-10 years), next most correct was age group II (11-20 years). The correct age estimation percentage diminished with the older age groups. The reliability reduced with the older age group. Age estimation in children and young patients are reliable but older age group further skeletal measurements are necessary.

    Introduction

    Materials and Methods

    In this retrospective study, OPG of 150 patients with known age ranging from 1 to 70 years, were collected. The OPG’s were taken at different private diagnostic center. Those OPGs were evaluated by two dentists. The age estimation were done according to decades. Then the estimated results were compared by the third dentists with the chronological age of the patients. Correct estimation was recorded as ‘true’ and wrong estimation was recorded as ‘false’. Patients were classified as :
    Group I- 1 -10 years
    Group II- 11 -20 years
    Group III- 21 -30 years
    Group IV- 31 -40 years
    Group V- 41 -50 years
    Group VI- 51 -60 years
    Group II- 61 -70 years

    The evaluation criteria of the evaluation were:

    1. Presence of primary teeth in mouth
    2. Mixed dentition period
    3. Presence of third molar teeth in mouth
    4. Apexigenesis and maturation of third molar
    5. Enamel attrition of teeth
    6. Width of root canal and pulp cavity
    7. Level of alveolar bone resorption

    The number and ratio of true and false predilections of two dentists were evaluated and differences among them were calculated using chi-square test.

    Results

    Both the dentist evaluated 75 OPGs each and predicted age of the patients. The first dentist predicted 81.33% correctly and the second one predicted 84% correctly. There was no significant differences between the dentists in terms of age estimation (p>0.05) The most accuracy in age determination was found in the range of 1-10 years (96.1% & 100%). The next most accuracy was found in the age group II (92.3% & 94.44%). The most incorrect age estimation occurred in the age group of 51-60 years, 61 and above age groups. The correct age estimation percentage diminished in the older age ranges.

    OPG A: This OPG shows presence of primary tooth (mixed dentition), Apexigenesis of permanent second molar, beginning of calcification of third molar and large pulp chambers.

    OPG B: Attrition of enamel, loss of a alveolar bone (periodontal pocket formation). All permanent tooth are well formed (upper third molars are missing or extracted).

    Discussion

    Age estimation is important in forensic medicine, clinical dentistry and archeology. Chronological age is important in most societies for admission during school, marriage and sometimes in sports for different age group specific. Tooth changes with time and there are many studies which have documented features of aging. Gustafson first developed dental method of age estimation based on six criteria: attrition, secondary dentine in pulp, cementum formation, root resroption, periodontal recession and root translucency.

    This methods can also be applied in living persons. OPGs can provide tools for person identification with age. It may become necessary in trials, marriages, sports and identification in natural disasters. It will play an important role in forensic dentistry.
    Tooth development is a continuous process but determining the end point correctly is difficult. Research is needed to determine the apex closure of tooth is needed. In this system one should remember that the standard deviation of age estimations are about 10-12 years.

    Conclusion

    Two dentists came from different academic backgrounds. But no significant difference in age estimation between the dentists were found. Age estimation through the evaluation of OPGs was found to be most correct for the first decade of life. But the method became unreliable from the fourth decades.

    Age estimation with OPGs can be used to make a significant percentage in areas of forensic medicine and forensic dentistry. However, in addition to measurements of teeth, skeletal measurements and examinations are necessary to achieve accurate and reliable age estimation.
